UPS problem suddenly stops working at random?

Anyone else having trouble with UPS rates getting displayed? Over the last week, we've noticed one transaction get through without any shipping on it.

So we started testing. We have the ship-in-cart mod installed. Usually, the first time you put something in the shopping cart, you will get no options for UPS. Then you hit refresh and will see the UPS rates.

Secondly, when you go to checkout, you will run into the same problem. We hit refresh once, no ups options displayed and no rates. hit refresh again and it displayed the ups rates and options.

We are using upsxml v1.3.4
